Dr. Faruqi is a licensed physician in Ontario, and this consultation is covered by his medical license in Ontario, Canada.
Initial Consultation - $260
Purpose of the Medical Consultation
This consultation is provided to offer personalized medical advice, diagnosis, or treatment tailored to your specific needs. It will complement your healthcare journey by addressing hormonal optimization, menopause management, or other related concerns.
During this session, Dr. Faruqi will:

This consultation establishes a doctor-patient relationship between you and Dr. Faruqi.

Privacy and Confidentiality

This session may be conducted via Zoom, a third-party platform with encryption to protect your privacy. However, no communication method is entirely secure, and confidentiality cannot be guaranteed.

This session will not be recorded unless explicitly agreed upon beforehand.
Client Responsibilities

You are responsible for providing accurate and complete information about your medical history, symptoms, and concerns to the best of your ability.

You are responsible for following up with Dr. Faruqi as advised and reporting any changes in your condition.
Limitations of Liability

Dr. Faruqi and Timeless Youth Empowerment assume no responsibility for outcomes resulting from inaccurate or incomplete information during the consultation.

Cancellation and Refund Policy

Cancellations or rescheduling must be made at least 24 hours in advance to avoid a $50 fee.

Refunds are not provided once the consultation has been completed.
